Your insurance declaration page, also known as the dec page, summarizes the information essential to your insurance coverage. It includes your name and address, descriptions of the insured property and your premium. It also outlines your policy's coverages, limits, deductibles, discounts and relevant insurance policy forms and endorsements.
Each separate insurance policy (homeowners, car, renters, etc.) will have a separate declaration page, usually located on the first page of the policy. Here’s a quick snapshot of what you’ll find on each page:
Named insured. At the top of the page, you will see the name and address of at least one named insured. The first person named is the primary named insured. There can be multiple named insured — these are the policyholders.
General policy information. At the top of the page, you’ll see the policy number, policy period and billing account number. Your policy number and billing account number are used to identify and pay for your specific policies, while your policy period states your policy’s effective and expiration dates.
Discounts. After your policy information, you’ll find any discounts applied to your policy. Don’t see any? Get in touch with your American Family Insurance agent to see if you’re eligible for any savings!
Insured property. Moving down, you’ll find information on your insured property. For example, an auto policy declaration page will include your vehicle information — make, model, year and vehicle identification number. A home or renter’s policy will include the address and type of residence.
Coverages and limits. The bulk of info in the middle of the page will highlight your coverages, limits and deductibles. You’ll also see the premium, or cost, associated with each level of coverage. Your total premium will appear below your policy number, policy period and billing account number.
Policy forms and endorsements. Some of your policies or endorsements will have forms that detail the specifics of your coverage. Those form numbers may appear on your declarations page as a point of reference for you and your agent.
Agent contact information. At the very bottom of your declarations page, you’ll find your American Family Insurance agent’s contact information. Have a question? Give them a call — they’re always happy to help.
